Message to U.S. Citizens: Visit by the Federal Benefits Unit from April 24-28, 2023 (March 29, 2023)

 

The U.S. Embassy in Guatemala is pleased to announce that representatives from the San Jose Regional Federal Benefits Office will visit Guatemala City to offer services for beneficiaries and for other individuals who have questions about U.S. Social Security benefits.

Where:  INGUAT (Instituto Guatemalteco de Turismo) Building, 7a. Avenida 1-17, First Floor, Zone 4

When:  April 24, 25, 26 and 27 (Monday through Thursday) from 8 am to 4 pm and April 28 (Friday) from 8 am to 1:30 pm

Walk-in Services:  No appointment necessary, BUT PLEASE BE PREPARED TO WAIT.

What to Bring:  Please see the list of services below.

Social Security Benefits:  Bring the original and one copy of the following documents for each applicant:  birth certificate and passport.  If applying for auxiliary benefits or survivor’s benefits, please also bring the original and one copy of your marriage certificate and/or spouse’s death certificate. 

Social Security Card Application:  Bring the original and one copy of the following documents for each applicant:  valid U.S. passport, Consular Report of Birth Abroad, and original birth certificate.  Please also bring the completed form SS-5FS.

Foreign Enforcement Questionnaire (FEQ):  This is known as the “proof of life” form.  These forms were mailed to individuals who have a mailing address in Guatemala and whose last two digits of their Social Security number under which they receive benefits are 00 through 49.

Federal Benefits Unit staff will be unable to accept forms for individuals whose last two digits of their Social Security number under which they receive benefits are 50 through 99.  Those individuals will receive these forms in the mail around June/July 2023.
