Passports for Minors Aged 15 and Under
The personal appearance of all minors under age 18 is required. The personal appearance of both parents to execute the passport application of a minor under aged 15 and under is required with limited exceptions. The usual period of validity of a passport for minors aged 15 and under is five years.
Passports for Minors Aged 16 and17
The personal appearance of all minors under age 18 is required. For minors aged 16 and 17, parental awareness by at least one parent is required. The usual period of validity of a passport for persons aged 16 and above is 10 years.
You must apply in person if at least ONE of the following is true: • You are applying for your first U.S. passport; OR • You are under age 16; OR • Your previous U.S. passport was issued when you were under age 16; OR • Your previous U.S. passport was lost, stolen, or damaged; OR • Your previous U.S. passport was issued more than 15 years ago.
We can issue a limited validity emergency passport on the same day. Emergency passports are generally valid for one year. Note that this emergency passport is not an e-Passport and does not contain an electronic chip. It is valid to travel to the United States, but it may not meet the requirements of other countries.
You may qualify to renew your passport by courier if you meet ALL the criteria.
Once your passport has been reported lost or stolen online, you may no longer use that passport even if you subsequently recover it.
Applicants Age 15 or Under Without Both Parents Present
The DS-3053 is the Statement of Consent for Issuing a Passport to a Child and the DS-5525 is the Statement of Special Family Circumstances.
Adult Passport Renewal (DS-82)
Renew an existing passport issued to you when you were aged 16 or older.
Report and Replace Lost/Stolen Passport (DS-64 & DS-11)
Report a passport lost or stolen and replace it.
You must provide one photo on a white background with your passport application. Our photo policies apply to adults and children. We have a few exceptions to our photo policies for infants.
